We require proof of eligibility as outlined by the Ministry of Health. If you are unsure, please refer to the information on the Health.govt website.

Terms of Trade
  • Payment for an appointment needs to be made on the day of your consultation.
  • Any invoices issued after a consultation need to be paid within 7 days of being used.
  • Charges may be made for telephone consultation, forms left for completion and other work completed by our doctors outside of consultation time.
  • If you wish to cancel your appointment, you must inform us at least 24 hours in advance.
  • We charge a fee of $20.00 if you fail to show up to your appointment.
Debt Collection
  • I understand that if I fail to make payments on my account, it may be given to a debt collection agency.
  • Please note that this action will cause extra costs for you.
  • If a credit limit or time for payment has been exceeded, you will be notifiedm and we will no longer provide medical services to you, unless it is an emergency.



Terms of Trade

  • Until proof of eligibility is provided you will be charged as per our visitor patient fees schedule. A copy of this is on display in our reception and on our website, collingwoodhealth.co.nz. 
  • Payment for all appointments is required on the day of your consultation.
